One such initiative was the Open Source Electronic Health Record Alliance (OSEHRA). Established by the US Department of Veterans Affairs (VA) around 2011, OSEHRA aimed to be the cornerstone of open-source software efforts in the health information technology community​. It focused on nurturing a vendor-neutral community engaged in advancing EHR software and related health information technology. Through this community, OSEHRA fostered the growth of an ecosystem of complementary products, capabilities, and services, characterized by transparency and agility, reminiscent of open-source software projects.

Among the key products developed under the OSEHRA initiative were the OSEHRA VistA and popHealth. OSEHRA VistA is an open-source EHR that is community-supported, customizable, and can be implemented by developers or providers in the healthcare industry. It maintains compatibility with the VistA EHR system developed by the VA, while also integrating enhancements from the wider commu​nity.

The popHealth, on the other hand, is an open-source quality measure reference implementation that aids healthcare providers in meaningful use quality measure reporting and supports easier submission of quality measures to public health organizations.

The shut down

However, the future of OSEHRA became uncertain as the VA and the Department of Defense, the largest users of VistA, decided to replace the system​. The organization faced a critical juncture where it had to expand beyond VistA or risk becoming irrelevant​​. Despite these challenges, OSEHRA continued to maintain and develop the VistA source tree, keeping it open for community contributions​.

Unfortunately, OSEHRA shut down in February 2020, with most of its assets, including web content, training materials, and journal content, transferred to WorldVistA. WorldVistA, founded to extend and collaboratively improve the VistA EHR and health information system for use outside its original setting, is continuing the mission of improving healthcare worldwide by making medical information technology better and universally affordable. It seeks to help those who choose to adopt the VistA system to successfully master, install, and maintain the software for their own use, fostering a community based on principles of open, collaborative, peer review software development and dissemination​.

The evolution of OSEHRA and its subsequent transition to WorldVistA exemplifies the dynamic and adaptable nature of open-source initiatives. Despite the hurdles, the commitment to drive innovation, ensure freedom from vendor lock-in, lower total cost of ownership, and enhance health information technology interoperability remains steadfast. Open source is indeed the wind of innovation that is constantly shifting the software development business model from selling proprietary software packages to providing software based on vibrant innovation, epitomizing the essence of open-source methodologies.
